Billing & Insurance

We participate with most but not all insurance plans. Due to wide variations in plans and benefits, we check with your insurance company, whenever possible, to verify your coverage prior to your visit. This helps us and you understand what to expect. Our goal is to provide rational, cost-effective and comprehensive care.

If you provide complete and accurate information about your insurance, we will submit claims to your insurance carrier and receive payments from them for our services. Ultimately, it is your responsibility to understand whether your insurance has limits on the doctors you can see, or the services you can receive. Depending on your insurance coverage, you may be responsible for co-payments, co-insurance, or other deductible amounts. Billing can be complicated, and we will work with you to help you understand how your insurance company or Medicare processes your bill.

To expedite your visit, you may download and complete the first visit forms. You may give us your insurance information by phone at (516) 616-0808extension 104, or by fax at (516) 616-0993. We will need copies of your insurance card(s) to process your claim. You are encouraged to use our patient portal for HIPAA compliant correspondence.
